Bulking Agents Appear Effective for Stress Urinary Incontinence Over Long Term


NEW YORK (Reuters Health) – Bulking agents appear effective long-term for treating stress urinary incontinence in older people, researchers from Italy report.

“Bulking agents represent a noninvasive procedure for the treatment of stress urinary incontinence (SUI) in all patients where a more invasive procedure may increase perioperative risks and complications,” Dr. Francesco Plotti from the University of Rome “Campus Bio-Medico” told Reuters Health by email.

Midurethral sling placement is the gold-standard treatment for SUI, but many patients do not wish to undergo invasive surgery, and some (including older patients) are at increased risk for anesthetic complications. For them, urethral bulking agents are a minimally invasive alternative.

Dr. Plotti’s team evaluated the long-term efficacy and patient satisfaction rate in a retrospective study of 63 patients aged 56 to 91 years who had urethral bulking-agent implantation for SUI.

At the time of follow-up, 15 patients (24%) remained cured, 12 (19%) remained improved, and 36 (57%) had failed treatment, the researchers report in Menopause, online March 19.

Eleven patients had developed new urinary symptoms (eight reported urgency, three nocturia).

Among patients treated less than 7.8 years ago (the median), the overall success rate was 42%; among those treated earlier, the overall success rate was 44%.

These two groups did not differ significantly with regard to onset of new symptoms or subjective satisfaction rates (24% were very satisfied and 19% were satisfied with the result).

“We can treat incontinence also in older patients in order to improve quality of life,” Dr. Plotti concluded.
